how can I make another meal preference block with check boxes where guests can chose which wine, accompaniments and salad dressing they want?  Thank you.

  You can easily do this by dragging and dropping a new "meal preferences" block to your event form from the left sidebar menu. This detailed guide can help you out with that: How to add, edit or delete a form block?
